Our client, a Non-Profit Healthcare Organization in Hartford, CT, is in need of a Nurse Coordinator for a full-time position. Shift for this role is Monday – Friday, flexible schedule (40 hours per week). Weekend Call – every 6 to 8 weeks. $300 stipend to take call.  

Nurse Coordinator Minimum Requirements:

  • Current Connecticut Registered Nurse license; BSN, professional nursing experience
  • Current Connecticut driver’s license: Will need to travel between the 3 homes you are coordinating for in the Hartford area

Nurse Coordinator
The Group Home Nurse Coordinator ensures the provision of all health services for program participants and follows the directions of medical providers to maintain participant’s health and well-being, in keeping with nursing standards, DDS/DPH regulations and corporate mission. The Group Home Nurse Coordinator works closely with participants, parents, guardians, supervisors and department heads, direct support staff and other   employees, labor union representatives, behaviorist, SLP, PT, Psychiatrists, doctors, state and federal regulators, vendors, suppliers

Duties and Responsibilities: 
 

  • The RN Coordinator works in conjunction with the Program Manager and Supervisor, reporting to the RN Supervisor. While this position does not include direct employee supervision, LPNs and direct care employees work under the direction and license of the RN about medical matters.
  • Secures medical, dental, pharmacy and related health care services in the community, and works with health professionals maintaining regular contact, also works with families and vendors, to remain informed of changes which might affect services for participants.
  • Communicates problems and issues relating to the health of participant’s and follows up in writing until closure to ensure that these are addressed in a timely manner. Represents   in a positive and professional manner when interacting with participants, co-workers, families, outside agencies, and others in the community; ensure that all actions, activities, and decisions are aligned with   vision and values; creates a welcoming, attractive, person-centered, culturally aware environment for participants and staff.
  • Advises/assists supervisors with medical/health-related appointment schedules (regular appointments as well as referrals/recommendations); prepares appropriate forms and documents follow-up, to facilitate the maintenance of participants' health. Accompanies participants on special medical appointments, when necessary
  • Participates in fact findings regarding injury, medication violations and suspected abuse or neglect, as needed, to ensure that the health-related aspects of such issues are properly addressed.
  • Participates in Planning and Support Team meetings regarding pre-admission and discharge planning, quarterly and annual reviews/updating of participants' programs, to exchange information pertinent to their physical and mental health.
  • The RN will assess and monitor the health status of participants on an on-going basis, following the orders of the physician, with the assistance of the Manager, Program Supervisor, and direct care staff, as a means of ensuring optimal health. Assesses and reviews the nutritional status of each participant quarterly; consulting with the dietitian to ensure that participants are given appropriate diets.
  • Interacts with participants on an on-going basis to better understand their needs, likes, dislikes, and personalities, while developing familiarity with their individual health issues, thereby providing better, personalized service.
  • Assists in developing and monitoring self-administration of medication program as addressed by the physician, to ensure that participants are allowed the opportunity to develop self-reliance in the taking of their medication.
  • Fully utilizes electronic health records to review, document and monitor the participant’s health. Creates quarterly and annual health care summaries and submits these to the Planning and Support Team prior to IP/OPS meeting, to ensure input on health-related issues. Reports communicable diseases to the RN supervisor so that proper prevention measures can be taken.
  • Teaches support staff and participants in health-related issues including safe storage, handling, and administration of medications; health care needs, and personal care / hygiene; in order that they have the information and delegation required to ensure that participants maintain good health. 
  • Reviews and initials all incoming medical/health related information including physicians'/dentists' notes; lab reports, and any special tests, to maintain awareness regarding such information. Follows up with medical professional as necessary. Requests and receives nursing discharge summary and physicians’ orders when a participant is discharged from the hospital, to ensure that support staff and supervisors are aware of the type of follow up care needed.
  • Monitors medication administration, incident reporting, restraint, and other health related reports, following up with Planning and Support Team members, to ensure that appropriate measures are taken where necessary.
  • Checks Medication Administration Record (MAR) and medication cabinets regularly; notes and acts on any Medication Administration Violations, notes labels, expiration dates and discontinued meds, to ensure that correct procedures regarding medications are being adhered to. Sets up and maintains accurate medical records for participants to ensure that such records are complete and current, meets policy guidelines, and DDS and/or ICF regulations.
  • Administers the practical portions, (Lab Practicum and On-Site med passes) of the Medication Certification Program, provides any teaching mandated by DDS in the CUP recertification process, and provides any additional teaching, to ensure that support staff are properly prepared for dispensing medication to participants.
  • Reviews medical records routinely to ensure that medication administration and documentation are in accordance with DDS and Standards. Addresses concerns about support staff who fail to administer medication in accordance with their CNP training in accordance with Medication Administration Policy and DDS regulations and sanction process. 
  • In ICF locations reviews medications every 90 days and coordinates with pharmacist in completing quarterly drug regimen reviews for each participant; signs/dates review; ensures that the reviewing physician's signature is in place and follows through on recommendations and/or any apparent irregularities. Performs quarterly visual assessments on each participant, documents and refers as needed, to ensure that the health needs of the participants are met.
  • Is on call to accept calls to advise on participant’s health matters.
