Key Responsibilities

  • Conduct comprehensive assessments of patient health status and monitor ongoing medical and behavioral needs
  • Administer medications, treatments, and routine nursing procedures as prescribed
  • Develop and implement individualized care plans in collaboration with healthcare providers
  • Educate patients and caregivers on health management, medications, and preventive care
  • Coordinate care effectively with physicians, therapists, social workers, and other healthcare professionals
  • Maintain accurate patient records and clinical documentation in compliance with healthcare regulations
  • Monitor patient progress and promptly report any changes in condition
  • Ensure adherence to safety standards, healthcare regulations, and organizational policies

Required Skills & Qualifications

  • Active RN license in Connecticut or applicable state
  • Associate’s or Bachelor’s degree in Nursing (ADN or BSN)
  • 1–3 years of nursing experience preferred, particularly in community health, outpatient, long-term care, or similar settings
  • Strong clinical assessment and care coordination skills
  • Knowledge of chronic disease management and patient education techniques
  • Excellent communication, organizational, and teamwork abilities
