Key Responsibilities
- Conduct comprehensive assessments of patient health status and monitor ongoing medical and behavioral needs
- Administer medications, treatments, and routine nursing procedures as prescribed
- Develop and implement individualized care plans in collaboration with healthcare providers
- Educate patients and caregivers on health management, medications, and preventive care
- Coordinate care effectively with physicians, therapists, social workers, and other healthcare professionals
- Maintain accurate patient records and clinical documentation in compliance with healthcare regulations
- Monitor patient progress and promptly report any changes in condition
- Ensure adherence to safety standards, healthcare regulations, and organizational policies
Required Skills & Qualifications
- Active RN license in Connecticut or applicable state
- Associate’s or Bachelor’s degree in Nursing (ADN or BSN)
- 1–3 years of nursing experience preferred, particularly in community health, outpatient, long-term care, or similar settings
- Strong clinical assessment and care coordination skills
- Knowledge of chronic disease management and patient education techniques
- Excellent communication, organizational, and teamwork abilities
